Specialized Depression Treatment

For adults living with depression that has not improved through traditional methods, Pine Grove offers advanced options backed by medical expertise and compassionate care.

Our team specializes in treating those with complex or treatment-resistant depression using two leading interventions: Transcranial Magnetic Stimulation (TMS) and Spravato® nasal spray. These services are designed for people who need something greater than standard antidepressant medications can offer.

Treatment-Resistant Depression

Depression is a serious medical condition that affects how a person feels, thinks, and functions in everyday life. For many people, traditional treatment plans involving antidepressant medications and therapy lead to significant improvement. However, some may continue to experience persistent symptoms despite trying multiple medications and treatment approaches.

This is called treatment-resistant depression (TRD). A diagnosis of TRD may apply if:

  • You have tried at least two different antidepressants at adequate doses for a sufficient length of time without improvement
  • You have experienced intolerable side effects from medication that prevented you from continuing treatment
  • Your symptoms return quickly after initial improvement

TRD is more common than many realize, and it can be frustrating and exhausting. Pine Grove’s Outpatient Services team offers evidence-based treatments specifically designed to help people living with TRD gain relief and improve their quality of life.

Who Benefits From Specialized Depression Treatment

Our specialized depression treatment services are intended for adults aged 18 and older who:

  • Have been diagnosed with major depressive disorder
  • Have not experienced lasting improvement from antidepressant medications
  • Are seeking alternatives to traditional medications due to limited results or difficult side effects
  • May be looking for options that do not require daily medication use
  • Have completed prior evaluations and are open to non-invasive or supervised clinical treatments

Patients interested in these services will receive a thorough evaluation by a Pine Grove provider to determine whether TMS or Spravato® is appropriate. We assess current and past treatments, medical history, and overall goals for care. In some cases, patients receiving other mental health services at Pine Grove may be referred internally for these treatments if clinically indicated.

Transcranial Magnetic Stimulation: TMS for Depression

TMS is a non-invasive treatment that uses magnetic pulses to stimulate specific areas of the brain known to influence mood. It is approved by the FDA for treatment-resistant depression and does not involve medication or sedation.

A small device is placed against the patient’s head to deliver targeted magnetic pulses. These pulses activate neurons in areas of the brain linked to mood regulation. TMS is conducted while the patient is seated in a comfortable chair, and patients are awake and alert during the procedure. Here’s what to expect:

  • Your first visit includes a 90-minute evaluation and a brief orientation
  • The first treatment session is approximately one hour and includes brain mapping
  • Subsequent sessions last around 30 minutes
  • TMS is typically delivered five days a week for six weeks, followed by a gradual reduction in frequency over three additional weeks, totaling about 36 sessions
  • Patients may return to work and normal activities after each session

Most patients describe the sensation as light tapping on the scalp, along with a clicking sound. Side effects may include mild scalp discomfort, headaches, or facial muscle twitching. These are completely normal and usually short-lived and decrease over time.

Spravato® (Esketamine Nasal Spray)

Spravato® is an FDA-approved medication for adults with treatment-resistant depression. It is administered as a nasal spray under direct supervision at Pine Grove Outpatient Services and is used in combination with an oral antidepressant.

Spravato® contains esketamine, which affects glutamate receptors in the brain. This mechanism is different from traditional antidepressants and may offer fast-acting relief for some who have not responded to other options.

  • The first appointment includes a 90-minute evaluation and review of the treatment process
  • Initial dosing takes place twice a week for four weeks
  • After the first month, dosing may be reduced to once a week or every other week depending on your progress
  • Each treatment session lasts approximately 2.5 hours, including mandatory monitoring after administration

Because of potential minor side effects like sedation or dissociation, patients are required to remain at the clinic for two hours after treatment and must arrange for transportation home. Most side effects resolve the same day and become less frequent with continued use.

You may bring comforting items such as headphones, a blanket, or a book. Eating should be avoided two hours before treatment, and liquids should be avoided for 30 minutes beforehand. Our staff will review all medication use, including controlled substances and nasal sprays, to make sure treatment is delivered safely.
