As a new patient in our clinic our goal is to provide you with an accurate and comprehensive assessment. To accomplish this it is vitally important for us to obtain information from other healthcare providers and other organizations who have served you in the past.  Therefore, we need your help in obtaining the following information prior to your appointment in our clinic.  Please begin now to request these records to be sent to our office.  You may have them faxed to us at 601-288-4921 or mailed to the address below.

New Patient

Welcome and thank you for choosing our clinic.  We are glad you have made the important step in meeting your behavioral healthcare needs. 

As a convenience to our patients we offer the opportunity for you to review our policies and complete new patient paperwork at home prior to coming to our office for your first visit.  Please review all the information prior to your visit as well as print and complete a copy of the new patient paperwork located under “Patient Forms”. 

Please bring the following to your first appointment

On Time Arrival

So that you may take advantage of the full amount of time reserved for you by our clinic, please arrive at least 30 minutes early for your first appointment and 15 minutes early to subsequent appointments.  This will allow time for any necessary paperwork and triage by the nurse prior to being seen by the provider.  If you arrive late to your appointment, we may find it necessary to reschedule your appointment so that those patients behind you may be seen on time.

For Child & Adolescent Patients:

Toys, Books & Other Items
To avoid the spread of infectious diseases, our clinic does not provide toys, books, etc.    Please bring toys, books, or other things that would be appropriate for your child to keep them occupied while waiting. 

Consent for Treatment
The legal guardian for the child or adolescent patient must be present at the first appointment to sign the consent for treatment.  If the guardian does not wish to accompany the patient to subsequent visits, he or she should sign a consent form that will allow the doctor to see the patient with another responsible person.  If the legal guardian is someone other than that patient’s parent, the Court Order appointing the legal guardian must be brought to the first visit.

If the patient is under any type of Court Order, please provide a copy to the staff at the time of the first visit.

Take US Hwy 49 North
Bear right on to Broadway Drive / US-11 Hwy Exit
Merge onto Broadway Drive
Stay in the right lane for 0.3 miles
At the 1st red light (Broadway Drive and Lincoln Road) Turn Right
At the 4th red light after turning on to Lincoln Road you will be at intersection on Lincoln Road and 40th Ave.
Continue straight on Lincoln Road going over Interstate 59 for ..8 miles.
The Lincoln Center will be a 3-story building on the left.
Park and come in the front door entrance.

From Laurel
Take I-59 South towards Hattiesburg
Take the US-98 W / Hardy Street Ext – Exit 65
Take the Hardy Street ramp toward University of Southern Mississippi
Turn Left onto Hardy Street
Proceed to get into the right hand turn lane at first red light (Hardy Street & 40th Avenue)
Turn right onto 40th Avenue
At the 3rd stop light (40th Avenue and Lincoln Road) turn right onto Lincoln Road
Continue straight on Lincoln Road going over Interstate 59 for approximately 1 mile

From Columbia
Take US-98 East towards Hattiesburg  (Note: US 98 E turns into Hardy Street)
Turn right onto 40th Avenue (Notes: this is the first right after crossing over interested 59)
At the 3rd stop light (40th Avenue and Lincoln Road) turn right onto Lincoln Road
Continue straight on Lincoln Road going over Interstate 59 for approximately 1 mile
The Lincoln Center will be a 3-story building on the left.
Park and come in the front door entrance.

From Jackson
Take US 49 South towards Hattiesburg for approximately 82 miles
Merge onto I-59 South toward New Orleans / Gulf Coast and travel for 2.2 miles
Take the US-98 W / Hardy Street Exit – Exit 65
Take the Hardy Street ramp toward University of Southern Mississippi
Turn Left onto Hardy Street
Proceed to get into the right hand turn lane at first red light (Hardy Street & 40th Avenue)
Turn right onto 40th Avenue

To Find Our Suite:  Enter through the front door of the Lincoln Center.  As you walk through the Atrium, the Elevators are located on your left.  Come to the 2nd Floor.  Once on the 2nd floor, walk off the elevator and turn left.  Our suites are located along the left hand wall.

Clinic Policies

No shows

If you must cancel or reschedule your appointment, please call 288-8050 at least 24 hours in advance.   Your prompt notice allows patients in crisis or on our waiting list the opportunity to see a doctor quickly. Our answering service will take your call twenty-four hours a day, and notify us of your cancellation. 

Failure to provide at least 24 hours notice will result in the patient being charged a $35 fee which is payable prior to seeing your provider at the next visit.

We understand that there are occasional and unavoidable circumstances that require a late cancellation.  However, we hope you understand that consistent application of this policy is necessary. 

Medication Refill

Taking your medication exactly as it has been prescribed it is an important part of your care.  If you find that you are going to run out of your medication before your next appointment, please call our office at (601) 288-8065 at least 7 days before you run out. You will be asked to leave identifying information about the refill needed.  Please have your medication bottle in hand when making the call so that you will have the information needed.   Prescription refills may be dispensed at the discretion of the doctor or Nurse Practitioner.  As our clinic schedules vary, we will make every effort to review your prescription within 2 business days of your call.

Medication Refills are provided during office hours only and not on weekends or after hours. 

Request Records

After receiving a valid Authorization for Disclosure of Health Information, we will gladly forward a copy of your records to another licensed professional.  Requests for a copy of your medical record to be released to you or other individual entity will be reviewed on an individual basis.  The fee for this service includes the cost of preparation as well copying and mailing the record.

Privacy/Confidentiality

Communications between the patient and the healthcare provider are confidential and can only be released with the written permission of the patient.  If you would like for the doctor or office staff to communicate with someone else (doctor, spouse, parent, child, friend, employer, etc.) about any part of your care, including your bill or your appointments, please tell the registration staff.  We will request that you sign an authorization for release of information. 

There are limits to patient confidentiality.  The following circumstances require that the providers or his/her designee contact an appropriate outside party.

  1. Abuse/neglect of a minor child or impaired adult,
  2. If you threaten to harm another individual,
  3. If it is believed that you intend to harm yourself,
  4. When a court order or subpoena requires the release of information

 

Concerns & Compliments

Creating an exceptional patient experience for you is our priority and we invite your comments, suggestions, & compliments.  If you have comments or concerns about your care received during your visit, please take a few minutes to speak with the office manager before you leave.  If the manager cannot resolve the issue, or if you are not satisfied, you may contact the Director at 601-288-2584 or the Pine Grove Patient Advocate at 288-4951.

After your visit to our clinic, you may receive a survey in the mail at home asking questions about your experience in our clinic.  We would appreciate your taking the time to complete and return the survey in the postage paid envelope that is included.  Your feedback is vitally important to us and will help us to improve our services.

Payment Information

Payment Policy

Payment is due at the time of service.  This is inclusive of any patient due balances and copayments.  If you are unable to pay in full, please contact our Patient Accounts office at (601) 288-8064 to make payment arrangements. 

Insurance: 

As a courtesy to you we will gladly verify your benefits and file your insurance for you.  In order for our clinic to efficiently file your claim, please provide our registration staff with complete information regarding insurance, Medicare and/or Medicaid coverage at the time your initial appointment is made. 

FGH employees may have co-pays deducted from their paychecks by requesting a payroll deduction enrollment form.

Worker’s Comp & EAP

If your care is being paid for by Worker’s Comp or through an Employee Assistance Program (EAP), please notify the registration staff prior to seeing your provider.
