The Pine Grove Alumni Relations Department’s mission is “To provide continuous and up to date information for all program alumni as a tool for their recovery journey.” As an alumnus, you can help us fulfill our Mission by taking a few minutes to complete our on-line Pine Grove Behavioral Health & Addiction Services Alumni Survey. At the end of the survey you will be asked your name and some other identifying information so we can verify your status as a Pine Grove Behavioral Health and Addiction Services alumnus. This information will be sent through a secure server and will only be viewed by Pine Grove Alumni Services staff. If you wish to answer the questions anonymously or off-line, you can download a copy of the survey, complete it, print it, and mail or fax the survey to Alumni Services.
